Complete Forms W-4 and L-4 and return to your employer so that the correct Federal and Louisiana income taxes are withheld from your pay. Because your tax situation may change, you may want to recompute your withholding each year.

All employees, citizens, and non-citizens, hired after November 6, 1986, must complete Section 1 of this form at the time of hire, which is the actual beginning of employment. The employer is responsible for ensuring that Section 1 is timely and properly completed.

A Separation Notice Alleging Disqualification should be completed for each employee who leaves employment without good cause connected with that employee's work, is discharged for misconduct connected with work performance, or is unemployed because of a labor dispute.
